          INSTRUCTION TO THE APPLICANTS WHO APPLY FOR RELIEF UNDER CMRF

          • The applicant should have a BPL, ration card.
          • The applicant should have Aadhar number. The details like, name, father’s name, date of birth. Age, sex, address etc are considered as in the Aadhar card.
          • The applicant or the mother of a minor applicant should have a bank account linked to his /her Aadhar number.
          • The applicant should provide an active mobile number for communication.
          • The final bills in original are to be compulsorily submitted if the treatment is completed.
          • The bills issued within one year from the date of submission of the application are only considered.
          • The doctor who has treated the applicant must have signed the final bill with seal.
          • The bills of the medical stores are not considered. The bills of the labs and the purchase bills of costly drugs must be certified and counter- signed by the doctor with seal.
          • The discharge summery of the hospital where the treatment is taken is only considered with the seal and signature of the doctor. In case if on-going treatment for the diseases like cancer(chemotherapy) and kidney (dialysis), the discharge summary is not mandatory. But in such cases the doctor has to certify that the patient was treated in O.P.D.
          • The hospital which issues Estimate should have a e-mail id and phone number for further communication.
          • The applicant must not be a beneficiary of any other schemes or insurance companies.
          • Provision is made generally to get relief from CMRF only once and repeated applications are not entertained.
          • The application must be alive on the date of application. Applications from the legal heirs of the deceased patient are summarily rejected.
          • The R.T.A, M.L.C and cases pending in any court or regulatory authority are not entertained and considered for relief under CMRF.

          • The applicant should have a BPL, ration card.

          • If the treatment is yet to be taken, then the original estimate from the hospital has to be submitted.
          • The estimate issued must have been submitted with in 40days.
          • The hospital which issues Estimate should have a e-mail id and phone number for further communication.
          • The applicant must not be a beneficiary of any other schemes or insurance companies.
          • Provision is made generally to get relief from CMRF only once and repeated applications are not entertained.
          • The application must be alive on the date of application. Applications from the legal heirs of the deceased patient are summarily rejected.

          • The applicant has to give the full details of the tragic incident
          • The application has to be addressed to the Hon’ble Chief Minister
          • The applicant is expected to be of Economically backward
          • The applicant should have Aadhar number. The details like, name, father’s name, date of birth. Age, sex, address etc are considered as in the Aadhar card.
          • The applicant or the mother of a minor applicant should have a bank account linked to his /her Aadhar number.
          • The applicant should provide an active mobile number for communication.
          • Identity Proof
          • Identity Proof of Victim (In case you are availaing assistance on behalf of someone else)
          • Address Proof
          • Passport Size Photo (35*45)
          • BPL card (front & Back)
          • Hospital Bills
          • Death Certificate
          • Recommendation Letter
          • Any other relevant documents
